Terms and Conditions for Membership

FIT4MOM SUNNYVALE-MOUNTAIN VIEW MONTHLY MEMBERSHIP AGREEMENT

This contract is made between the client and FIT4MOM Sunnyvale-Mountain View-San Jose and is a one-time agreement. This membership allows you to attend unlimited and all programmed classes. Non-refundable. Non-transferrable.

AUTHORIZATION FOR PAYMENT

I authorize FIT4MOM Sunnyvale-Mountain View to initiate a charge to the account entered into Pike 13 for my payment.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Sunnyvale-Mountain View to fulfill my payment commitment and I am obligated to pay. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend classes who have unpaid balances. If a membership remains unpaid it will be canceled.

CANCELLATION POLICY

No refunds will be given for class passes once it has been paid. This class pass is not transferable to another person or anywhere outside the Sunnyvale-Mountain View-San Jose franchise.

AGREEMENT

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with the proper advance written notice.
